One bugfix and a few tidy-ups:

 * Pipe fault logging was broken on Gen9+.
 * Removed some unnecessary local variables.
 * Removed unnecessary initializers.
 * Decreased pipe iir block indentation level.
 * Grouped variable initialization close to use sites.
