[PATCH 04/20] iomap: Rename fiemap_ctx to fiemap_iomap_ctx

fiemap_ctx structure is local to iomap code, so, rename it to match its
scope.

A new structure named fiemap_ctx will be added which will be globally
available as part of the fiemap kernel API.
