[PATCH 9 of 23] IB/ipath - remove stale references to userspace SMA

["Bryan O'Sullivan" <bos@pathscale.com>]

When we first submitted a userspace subnet management agent, it was
rejected, so we left it out of the final driver submission.  This patch
removes a number of vestigial references to it.
