It's hard to reproduce. It seems we have a fix but I misunderstood some parts of the kernel page allocation scheme and, so, I am not entirely the root issue is well analyzed. This patch can wait until I grab a larger system with 2TB. I will send a v2. Greg had some valuable comments on extra cleanups. Thanks, C.
