Hello,

This is ported to latest upstream. This update also adds a max_ptes_none sysfs
control for khugepaged. It also registers khugepaged on vma_merge to allow
khugepaged to collapse hugepages as the app extends vmas on already faulted
areas. It adds a test_and_set_bit in __khugepaged_enter (set_bit wasn't
enough), found by review only.
