Subject: [PATCH 1/2] man: fix mandoc warnings

They are:
 * prologue macros out of order: Dt after Os
 * skipping paragraph macro: Pp before Bd
 * whitespace at end of input line
 * skipping empty macro: Cm
 * skipping paragraph macro: Pp before It
 * sections out of conventional order: Sh ENVIRONMENT
 * skipping -width argument: Bl -item
