I have not been able to find any integration issue in the kernel/qemu code but maybe I am too blind now as I wrote it ;-) When I get a guest stage1 TLBI I cascade it down to the physical IOMMU. I also pass the LEAF flag. As you are an expert of the SMMUv3 PMU, if your implementation has any and you have cycles to look at this, it would be helpful to run it and see if something weird gets highlighted.
