Hi all,

This series clean up the block polling code a bit and changes the interface
to poll for a specific bio instead of a request_queue and cookie pair.

Polling for the bio itself leads to a few advantages:

  - the cookie construction can made entirely private in blk-mq.c
  - the caller does not need to remember the request_queue and cookie
    separately and thus sidesteps their lifetime issues
  - keeping the device and the cookie inside the bio allows to trivially
    support polling BIOs remapping by stacking drivers
  - a lot of code to propagate the cookie back up the submission path can
    removed entirely

The one major caveat is that this requires RCU freeing polled BIOs to make
sure the bio that contains the polling information is still alive when
io_uring tries to poll it through the iocb. For synchronous polling all the
callers have a bio reference anyway, so this is not an issue.

   slabs bio RCU
 - split the spin argument to bio_poll to avoid sleeping under RCU from
   io_uring
 - add support for polling nvme multipath devices

The polling support in the legacy direct-io support is a little crufty.
It already doesn't support the asynchronous polling needed for io_uring
polling, and is hard to adopt to upcoming changes in the polling
interfaces.  Given that all the major file systems already use the iomap
direct I/O code, just drop the polling support.

If an iocb is split into multiple bios we can't poll for both.  So don't
bother to even try to poll in that case.

There is no point in sleeping for the expected I/O completion timeout
in the io_uring async polling model as we never poll for a specific
I/O.  Split the boolean spin argument to blk_poll into a set of flags
to control sleeping and the oneshot behavior separately.

Unlike the RWF_HIPRI userspace ABI which is intentionally kept vague,

This flags ensures that the pages will not be reused for non-bio
allocations before the end of an RCU grace period.  With that we can
safely use a RCU lookup for bio polling as long as we are fine with
occasionally polling the wrong device.

'struct bvec_iter' is embedded into 'struct bio', define it as packed
so that we can get one extra 4bytes for other uses without expanding
bio.

'struct bvec_iter' is often allocated on stack, so making it packed
doesn't affect performance. Also I have run io_uring on both
nvme/null_blk, and not observe performance effect in this way.

Replace the blk_poll interface that requires the caller to keep a queue
and cookie from the submissions with polling based on the bio.

Polling for the bio itself leads to a few advantages:

 - the cookie construction can made entirely private in blk-mq.c
 - the caller does not need to remember the request_queue and cookie
   separately and thus sidesteps their lifetime issues
 - keeping the device and the cookie inside the bio allows to trivially
   support polling BIOs remapping by stacking drivers
 - a lot of code to propagate the cookie back up the submission path can
   be removed entirely.

The nvme multipathing code just dispatches bios to one of the blk-mq
based paths and never blocks on its own, so set QUEUE_FLAG_NOWAIT
to support REQ_NOWAIT bios.

After POLL_CAPABLE is enabled on nvme mpath, POLL can be disabled via
queue_poll_store(). However, blk_mq_freeze_queue() just blocks and drain bio
submission, then pending POLL bio can't be polled any more because
QUEUE_FLAG_POLL is checked in bio_poll().

the block device is RCU freed, so we are fine there.  There rest OTOH
is more interesting.  Let me think of a good defense using some kind
of liveness check.

The point of BLK_MQ_F_BLOCKING is that ->queue_rq can block, and
because of that it is not called from the submitting context but in
a work queue.  nvme-tcp also sets QUEUE_FLAG_NOWAIT, just like all blk-mq
drivers.

Or hold gendisk reference in bdev lifetime, then everything referred
won't be released until bdev is freed.

BLK_MQ_F_BLOCKING can be called from submitting context, see
blk_mq_try_issue_directly().

The all drivers setting it have a problem, as the blk-mq core sets
BLK_MQ_F_BLOCKING for all of them.  The right fix would be to not make
it block for REQ_NOWAIT I/O..

The whole device bdev controls the gendisk liftetime, so that one is
easy.  But for partitions it is probably a good idea to ensure that
the gendisk is kept allocated as long as the block devices are around
as well.

Looks we needn't to care if the bdev is disk or partition: bdev is always
associated with gendisk via ->bd_disk, the gendisk instance has to be kept
alive since bio->bi_bdev->bd_disk is used everywhere almost.

Hey Christoph and Ming,

I think that we can remove BLK_MQ_F_BLOCKING setting from nvme-tcp,
the reason that it was set it because lock_sock taken in sendpage is
taking the sk_lock mutex, however given that nvme_tcp_try_send is
protected with the queue send_mutex (which is taken with a
mutex_trylock) we can probably convert it to sendpage_locked...

Let me run it through some tests and get back to you...

Add a helper to get the hctx from a request_queue and cookie, and fold
