* Chasing IO errors. BTRFS: error (device dm-2) in btrfs_run_delayed_refs:2907: errno=-5 IO failure

Chasing IO errors.  BTRFS: error (device dm-2) in
btrfs_run_delayed_refs:2907: errno=-5 IO failure


I've just had an odd one.

Over the last few days I've noticed a file system blocking, if that is
the correct term, and this morning go read only.  This resulted in a lot
of checksum errors.

Having spotted the file system go read only in the logs and then noted
the error message in the subject shortly after booting I assumed a
hardware error and changed the SATA cable.  That had no effect so I
isolated the disk and mounted the respective file system degraded.
Shortly after mounting the degraded file system I had the same error
again. So I unmounted the file system edited fstab and swapped the disk
which I though originally had the error with the one now showing an error.

The file system is btrfs, kernel 5.2.9, RAID 1 with three WD reds of 3,
3 and 4 TB.  btrfs is on top of luks.

The original 'blocking' behaviour seemed to manifest itself as I
upgraded the kernel to 5.2.5 or 5.2.7 a day or two ago.  So I tried
5.1.21 to see if that made a difference when the error was showing.  It
did not.  Yesterday I had a backup with rsync, started early in the
morning that should take minutes to complete still running 8h later with
two CPU cores maxed.  Up until I had the file system go read only I had
not noticed anything amiss in the logs, but to be honest, I'd not looked
very hard.

smartctl did not show anything amiss with the drives.

Does this sound like a hardware error?  I have ordered a replacement
drive, if it is not needed as a replacement I will put it into a
homebrew NAS.

I've hit the issue again.  Hopefully the system is up long enough to
post this.

I'm a bit worried that trying to track this down disconnecting a disk at
a time I might hit the btrfs split brain issue.

That doesn't sound good. Checksum errors where? A complete start to
finish dmesg is most useful in this case.

OK but we don't know anything from what you've told us about what and
whose error, so it's all speculation. Definitely a complete dmesg is
needed.

Or if running systemd-journald to persistent media, you can look up
that boot with journalctl --list-boots, and export just the kernel
messages portion with something like this:

journalctl -b -2 -k -o -short-monotonic > journalbtrfshang.txt

That's two boots back, kernel messages only, monotonic time stamp.

Also useful if you experience blocked tasks, like a kind of system
hang for 2 minutes sort of thing, is a sysrq+t and the simple version
is, as root

# echo 1 > /proc/sys/kernel/sysrq
# echo w > /proc/sysrq-trigger
# echo t > /proc/sysrq-trigger

That will dump a bunch of task info into kernel messages, and will be
found in dmesg or the above journalctl command. It's useful to have
the echo 1 setup before you reproduce the problem; and even more
useful to use remote ssh to type out the 2nd command so all you have
to do is hit return upon reproducing the hang - otherwise it can take
a long time to type it all out.

WDC Reds have SCT ERC of I think 70 deciseconds by default which you
can check with 'smartctl -l scterc' for each drive. If it's hardware
related it probably isn't bad block related, and at least if the drive
is aware of the problem it'll report it via libata and you'll see such
messages in kernel messages.

This output should include a lot of info, like stack dump and several
different error message.

One single line with least amount of info is not going to help.

That run delayed refs failure mostly means extent tree corruption, or
some known fixed bug.

Please run btrfs check --readonly on that fs to see if it's corrupted.
If not then it's probably some runtime bug.

I've just booted, degraded, and the file system went readonly quickly.
Is it good to post the dmesg to here, it is 1341 lines?  I can do it
with all disks if that helps?  (or I could run it through grep, but I
suspect there is good reason for not doing that.)

I'll look at the sysrq stuff, but I'm not likely to hit that right this
moment as the system is not really usable in this state and I'm assuming
I'll do more harm than good soldiering on.

Can't do anything with the systemd stuff as I'm running slackware which
does not use it.

Also, this is really bad timing, so although I can do stuff with the
system for the next day or two I'll need to step away from it for a
short while.

Well, check will also report that transid mismatch, and possibly a lot
of extent tree corruption.

Not sure what the ECC part is doing, but it repeats quite some times.
I'd assume it's unrelated though.

This means, btrfs is trying to read extent tree for CoW, but at that
time, extent tree is already corrupted, thus it returns -EIO.

And btrfs_run_delayed_refs just returns error.

Not sure if it's related to device replace, but anyway the corruption
just happened.
The device replace may be an interesting clue, as currently our
dm-log-writes are mostly focused on single device usage.

Then I'd recommend to do regular rescue procedure:
- Try that skip_bg patchset if possible
  This provides the best salvage method so far, full subvolume
  available, although needs out-of-tree patches.

- btrfs-restore
  The regular unmounted recover, needs extra space. Latest btrfs-progs
  recommended.

Sorry, 'device replace'?  I've not done that lately.  I _may_ have tried
that years back with this file system.  However, iirc it failed as the
new, allegedly same size new disk was possibly slightly smaller.

From the above it looks like it is not a specific hardware failure.

I can give that a go, but not for a while.

I seem to be able to read the file system as is, as it goes read only.
But perhaps 'seems' is the operative word.

I've got the latest btrfs progs.    if neither of those two work I have
a backup.

So, basically, make a new file system and recover the data to it.  I've
a new disk on the way, so I can create a file system as single and once
I'm happy I've migrated data to it, wipe the old disks and move one  or
two to the new array and rebalance.

As long as you can mount RO, it shouldn't be mostly OK for data salvage.

Posting this for completeness, as this started just before I went away
for a while.

The filesystem was RO but there were failures copying affected files
meaning my copy from the RO file system was incomplete.  So I likely
_should_ have applied the patch suggested above, if that was my only
copy.  Instead I recovered from backups.
