IOMMU Fixes for Linux v5.9-rc5

Two fixes for the AMD IOMMU driver:

	- Fix a potential NULL-ptr dereference found by smatch

	- Fix interrupt remapping when a device is assigned to a guest
	  and AVIC is enabled.
