When target side trace in turned on and flush command is issued from the
host it results in the following Oops.

Fix this problem by adding a check for NULL namesapce in
drivers/nvme/target/trace.h:__assign_disk_name().

I think the problem is that the extra namespace lookup doesn't make
sense to start with.  We'll need to reuse req->ns, and for that move
the init tracepoint a bit:
