ppc_md.iounmap() is never set, drop it.
Once ppc_md.iounmap() is gone, iounmap() remains the only user of
__iounmap() and iounmap() does nothing else than calling __iounmap().
So drop iounmap() and make __iounmap() the new iounmap().
