Hi, Leon.
Can you test the attached patch?  If revert it, ib_qp_usecnt_dec() would called before ib_qp_usecnt_inc() when 
create_xrc_qp_user() return error.

There are NULL for pd,send_cq,recv_cq,srq in create_xrc_qp_user(), so
