Create contiguous physical memory regions by migrating/exchanging pages.

1. It scans all VMAs in one process and determines an anchor pair
(VPN, PFN) for each VMA.
2. Then, it migrates/exchanges pages in each VMA to make
them aligned with the anchor pair.

At the end of day, a physically contiguous region should be created for
each VMA in the physical address range [PFN, PFN + VMA size).
