Add missing MODULE_LICENSE() and MODULE_DESCRIPTION() in hsq driver to
fix below warning when compiling the hsq as a module.

"WARNING: modpost: missing MODULE_LICENSE() in drivers/mmc/host/mmc_hsq.o".
