Revert"x86/acpi: Enable MADT APIs to return disabled apicids"

Revert: 8ad893faf2ea ("x86/acpi: Enable MADT APIs to return disabled apicids")

Remove the leftovers of the boot time 'cpuid <-> nodeid' mapping approach.
