This patch improves code readability by changing the following construct:

>    if (cond)
>        goto passive;
>    foo();
>    goto out;
> passive:
>    bar();
> out:

into this:

>    if (cond)
>        bar();
>    else
>        foo();

as well as eliminating a few more goto statements and fixing header
includes.
