This doesn't appear to ensure uniqueness when using PCIe topologies:

$ ./build/x86_64-softmmu/qemu-system-x86_64 \
     -device virtio-net,acpi-index=100 \
     -device virtio-net,acpi-index=100
qemu-system-x86_64: -device virtio-net,acpi-index=100: a PCI device with acpi-index = 100 already exist

$ ./build/x86_64-softmmu/qemu-system-x86_64 \
     -M q35 \
     -device virtio-net,acpi-index=100
     -device virtio-net,acpi-index=100
....happily running....
