The mapping code is odd and looks broken. See FIXME in the comment.

Also fix the harmless off by one in the FIX_KMAP_END define.

Also fix the harmless off by one in the FIX_KMAP_END define.

Also fix the harmless off by one in the FIX_KMAP_END define.
