Hi,

These are few cleanups for the block trace.

Below is the test log on latest linux-block/for-next.

Last two patches are needed to for testing.

V1->V2:-

1. Add fixes tags.
2. Move EXPORT_TRACEPOINT_SYMBOL_GPL(blk_rq_insert) to
   block/blk-core.c.
3. Add reviewed-by tags.
4. Remove the null_blk patches fro now, create a separate series.
