Replace the dax_host_hash with an xarray indexed by the pointer value
of the gendisk, and require explicitl calls from the block drivers that
want to associate their gendisk with a dax_device.

Replace the dax_host_hash with an xarray indexed by the pointer value
of the gendisk, and require explicitl calls from the block drivers that
want to associate their gendisk with a dax_device.

Replace the dax_host_hash with an xarray indexed by the pointer value
of the gendisk, and require explicitl calls from the block drivers that
want to associate their gendisk with a dax_device.
