This has ll_rw_block users pass in the operation and flags separately,
so ll_rw_block can setup bio->bi_op and bio-bi_rw on the bio that
is submitted.

This has ll_rw_block users pass in the operation and flags separately,
so ll_rw_block can setup bio->bi_op and bio-bi_rw on the bio that
is submitted.
