Compared to v1, I have improved error recovery (thanks to Stefan
for the great review!).  Patch 3 is pretty much rewritten,
making patch 4 also much easier to review hopefully.

This makes little difference, but it makes the code change smaller
for the next patch that introduces MemoryRegionCache.  This is
because map/unmap are similar to MemoryRegionCache init/destroy.

the second time it is called.
