Please consider the following commit for longterm kernels 4.1 and 4.4:

s390/vmem: fix identity mapping
Strangely enough, it was already included in kernel 3.16.37, but not
in later branches, even though the fix only made it upstream in kernel
v4.7.

Sorry, I missed that one, now queued up for 4.4 and 3.18 stable (as if
anyone runs 3.18 on s390, but it can't hurt...)
