During my quest to switch block status to be byte-based, John
forced me to evaluate whether we have a situation during
copy-on-read where we could exceed BDRV_REQUEST_MAX_BYTES [1].
Sure enough, we have a number of pre-existing bugs in the
copy-on-read code.  Fix those (patch 3), along with adding a
test (patch 4, but needs a couple of additions in patch 1 and
2 to make the test even possible).  Swapping patch 3 and 4
demonstrates that the test indeed exposes flaws.

Improve our braindead copy-on-read implementation.  Pre-patch,
we have multiple issues:
- we create a bounce buffer and perform a write for the entire
request, even if the active image already has 99% of the
clusters occupied, and really only needs to copy-on-read the
remaining 1% of the clusters
- our bounce buffer was as large as the read request, and can
needlessly exhaust our memory by using double the memory of
the request size (the original request plus our bounce buffer),
rather than a capped maximum overhead beyond the original
- if a driver has a max_transfer limit, we are bypassing the
normal code in bdrv_aligned_preadv() that fragments to that
limit, and instead attempt to read the entire buffer from the
driver in one go, which some drivers may assert on
- a client can request a large request of nearly 2G such that
rounding the request out to cluster boundaries results in a
byte count larger than 2G.  While this cannot exceed 32 bits,
it DOES have some follow-on problems:
-- the call to bdrv_driver_pread() can assert for exceeding
BDRV_REQUEST_MAX_BYTES, if the driver is old and lacks
.bdrv_co_preadv
-- if the buffer is all zeroes, the subsequent call to
bdrv_co_do_pwrite_zeroes is a no-op due to a negative size,
which means we did not actually copy on read

Fix all of these issues by breaking up the action into a loop,
where each iteration is capped to sane limits.  Also, querying
the allocation status allows us to optimize: when data is
already present in the active layer, we don't need to bounce.

Note that the code has a telling comment that copy-on-read
should probably be a filter driver rather than a bolt-in hack
in io.c; but that remains a task for another day.

Add a test for qcow2 copy-on-read behavior, including exposure
for the just-fixed bugs.

The copy-on-read behavior is always to a qcow2 image, but the
test is careful to allow running with most image protocol/format
combos as the backing file being copied from (luks being the
exception, as it is harder to pass the right secret to all the
right places).  In fact, for './check nbd', this appears to be
the first time we've had a qcow2 image wrapping NBD, requiring
an additional line in _filter_img_create to match the similar
line in _filter_img_info.

Blah - I compiled with -g instead of -O2, which masks this warning in my
setup.

The warning is a false negative (the error message is actually pointing
to a line in bdrv_co_do_copy_on_readv - but the compiler must have
inlined it into bdrv_aligned_preadv) - the function is only ever called
with non-zero bytes, and therefore the 'while (cluster_bytes)' loop will
execute at least once, and ret always gets assigned.  But the compiler
can't see that, so I'll squash this in:

Yep, and I even thought about that, since I've been pointing it out on
other patches, but forgot to mention it when composing the email ;)

Probably :(  Although I didn't actually test that setup

But I don't know what other options we have to work around it. As long
as we are executing in the correct directory, I guess we can open both
BLKDBG_CONF and TEST_WRAP relative to ./ rather than as an absolute
path, and that should be sufficient to avoid spaces.  But I don't know
how easy that is to achieve, or if _filter_qemu_io will handle it correctly.

Maybe I take the wimpy way out and skip the test if $PWD contains spaces
or other problematic characters?

My thought is if the operation is destructive (writes, creates, deletes) a
file, we want to make sure we do not ruin someone's day by destroying a file
(e.g. "/home/jcody/qemu work", and having /home/jcody/qemu overwritten).

Spaces in pathnames for iotests don't work currently, however.  So maybe the
proper thing to do, is for me to add to my iotests series a check for spaces
in TEST_DIR, and just refuse to run any tests at all (with an informative
reason why).  And if it is ever fixed so that tests run (and run safely)
with spaces, we can remove that restriction.

Well, you could help the compiler with this:

    assert(cluster_bytes > 0);

Then it compiles. Unfortunately, the compiler was right and you weren't:

    $ ./qemu-io -C -c 'read 0 0' /tmp/test.qcow2

Of course - that's a good test to have.

So it turns out the reason we get in with 0 length is:

        ret = bdrv_is_allocated(bs, offset, bytes, &pnum);
        if (ret < 0) {
            goto out;
        }

        if (!ret || pnum != bytes) {
            ret = bdrv_co_do_copy_on_readv(child, offset, bytes, qiov);
            goto out;
        }

because bdrv_is_allocated() returns 0 for a 0-length query.  I wonder if
we should instead assert that bdrv_is_allocated/bdrv_get_block_status()
are given a non-zero size, and fix callers to avoid a 0-length query
(after all, it's tough to argue that the read is allocated in the
current layer or defers to a backing layer, when there is nothing to be
read).
