pagetable walks on address ranges mapped by VMAs can be done under the mmap
lock, the lock of an anon_vma attached to the VMA, or the lock of the VMA's
address_space. Only one of these needs to be held, and it does not need to
be held in exclusive mode.

Under those circumstances, the rules for concurrent access to page table
entries are:

 - Terminal page table entries (entries that don't point to another page
   table) can be arbitrarily changed under the page table lock, with the
   exception that they always need to be consistent for
   hardware page table walks and lockless_pages_from_mm().
   This includes that they can be changed into non-terminal entries.
 - Non-terminal page table entries (which point to another page table)
   can not be modified; readers are allowed to READ_ONCE() an entry, verify
   that it is non-terminal, and then assume that its value will stay as-is.

Retracting a page table involves modifying a non-terminal entry, so
page-table-level locks are insufficient to protect against concurrent
page table traversal; it requires taking all the higher-level locks under
which it is possible to start a page walk in the relevant range in
exclusive mode.

The collapse_huge_page() path for anonymous THP already follows this rule,
but the shmem/file THP path was getting it wrong, making it possible for
concurrent rmap-based operations to cause corruption.

Since commit 70cbc3cc78a99 ("mm: gup: fix the fast GUP race against THP
collapse"), the lockless_pages_from_mm() fastpath rechecks the pmd_t to
ensure that the page table was not removed by khugepaged in between.

However, lockless_pages_from_mm() still requires that the page table is not
concurrently freed. Fix it by sending IPIs (if the architecture uses
semi-RCU-style page table freeing) before freeing/reusing page tables.

Any codepath that zaps page table entries must invoke MMU notifiers to
ensure that secondary MMUs (like KVM) don't keep accessing pages which
aren't mapped anymore. Secondary MMUs don't hold their own references to
pages that are mirrored over, so failing to notify them can lead to page
use-after-free.

I'm marking this as addressing an issue introduced in commit f3f0e1d2150b
("khugepaged: add support of collapse for tmpfs/shmem pages"), but most of
the security impact of this only came in commit 27e1f8273113 ("khugepaged:
enable collapse pmd for pte-mapped THP"), which actually omitted flushes
for the removal of present PTEs, not just for the removal of empty page
tables.

That's an interesting point. For anon THPs, the page table won't get 
immediately freed, but instead will be deposited in the "pgtable list" 
stored alongside the THP.

 From there, it might get withdrawn (pgtable_trans_huge_withdraw()) and

a) Reused as a page table when splitting the THP. That should be fine, 
no garbage in it, simply a page table again.

b) Freed when zapping the THP (zap_deposited_table()). that would be bad.

... but I just realized that e.g., radix__pgtable_trans_huge_deposit 
uses actual page content to link the deposited page tables, which means 
we'd already storing garbage in there when depositing the page, not when 
freeing+reusing the page ....

Maybe worth adding to the description.

With CONFIG_MMU_GATHER_RCU_TABLE_FREE this will most certainly do the 
right thing. I assume with CONFIG_MMU_GATHER_RCU_TABLE_FREE, the 
assumption is that there will be an implicit IPI.

That implicit IPI has to happen before we deposit. I assume that is 
expected to happen during pmdp_collapse_flush() ?

This sounds sane and correct to me. No expert on file-THP, though.

For anon-THP it's the mmap lock and the rmap locks. I assume the only 
difference for file-THP is that the rmap lock is actually the mapping 
lock. Looking at rmap_walk_file(), that seems to be the case.


I wish at least PTE table removal could be done easier ... I already 
experimented some time ago with some ideas (e.g., lock in PMD table 
memmap) but it's all far from trivial and space in the memmap is rare.

Depends on the definition of "fine" - it will be a page table again,
but deposited page tables are not associated with a specific address,
so it might be reused at a different address. If GUP-fast on address A
races with a page table from address A being deposited and reused at
address B, and then GUP-fast returns something from address B, that's
not exactly great either.

Yeah, pmdp_collapse_flush() does a TLB flush, as the name says. And as
documented in a comment in mm/gup.c:

 * Before activating this code, please be aware that the following assumptions
 * are currently made:

 *  *) Either MMU_GATHER_RCU_TABLE_FREE is enabled, and
tlb_remove_table() is used to
 *  free pages containing page tables or TLB flushing requires IPI broadcast.

The "PMD changed" check should catch that. We only care about not 
dereferencing something that's garbage and not a page/folio if I 
remember the previous discussions on that correctly.

Yeah. You can also have private file VMAs that are associated with
both a mapping and a set of anon_vmas, and in that case you would need
to lock the mmap, the mapping, and the anon_vma root; but the file THP
code in khugepaged instead just bails on file VMAs with an anon_vma.

Page table reclaim and page table migration, where you might only have 
limited context and wouldn't want to take all these expensive locks in 
write mode (IOW, you wouldn't want to care about them at all).

You're on the CC list of my bug report to security@kernel.org
with title "khugepaged races with rmap-based zap, races with GUP-fast,
and fails to call MMU notifiers". That has an attached reproducer
thp_ro_no_notify_kvm.c that is able to read PAGE_POISON out of freed
file THP pages through KVM.
