1: NOP out most XPTI entry/exit code when it's not in use
2: disable XPTI when RDCL_NO
3: x86: log XPTI enabled status
4: use %r12 to write zero into xen_cr3
5: reduce .text.entry
6: enable interrupts earlier with XPTI disabled
7: also NOP out xen_cr3 restores of XPTI
8: avoid double CR3 reload when switching to guest user mode
