[1.] One line summary of the problem: Error 110 from ASMedia Host Controller

[2.] Full description of the problem/report: I'm seeing a failure from
XHCI_HCD when I stream video from Intel Realsense D435 cameras through
an ASMedia ASM3042 USB host controller. The issue usually manifests as
repeated Error 110s from the camera as long as I'm trying to stream
data:
[ 100.227800] usb 4-1: Failed to query (SET_CUR) UVC control 1 on
unit 3: -110 (exp. 1024).
Followed by a bit of a lockup from XHCI_HCD. lsusb will hang and I
can't get any image data from the camera. This problem seems to happen
sooner when there are multiple cameras connected and streaming. In the
logs below, I'm streaming from four cameras, two connected to an
ASM3042 and two via an Intel host controller. It seems to happen when
I stop and re-start streaming from the cameras repeatedly. dmesg and
tracing output are located in this folder:
https://bostondynamics1.box.com/s/qtn28it8avda6pvve5sowyaeff4jzlyr

[3.] Keywords (i.e., modules, networking, kernel): xhci_hcd, ASMedia,
ASM3042, Intel Realsense

[4.] Kernel version (from /proc/version): 6.1.0-rc8

[5.] Output of Oops.. message (if applicable) with symbolic information
     resolved (see Documentation/oops-tracing.txt): N/A

[6.] A small shell script or example program which triggers the
problem (if possible): This requires the ASMedia host controller
and Intel Realsense cameras to reproduce.

[7.] Environment
[7.1.] Software (add the output of the ver_linux script here):
If some fields are empty or look unusual you may have an old version.
Compare to the current minimal requirements in Documentation/Changes.

Had a quick look at the trace.
The control transfer that times out is queued at:

95.030596: xhci_urb_enqueue: ep0out-control: urb 000000005be6faad pipe 2147484160 slot 1 length 0/1024 sgs 0/0 stream 0 flags 00110000
95.030597: xhci_queue_trb: CTRL: bRequestType 21 bRequest 01 wValue 0100 wIndex 0300 wLength 1024 length 8 TD size 0 intr

It never completes so it's cancelled after 5 seconds.
xhci driver stops the endpoint to remove the cancelled transfer.

100.268771: xhci_urb_dequeue: ep0out-control: urb 000000001ac66029 pipe 2147484160 slot 1 length 0/1024 sgs 0/0 stream 0 flags 00110000
100.268797: xhci_dbg_cancel_urb: Cancel URB 000000001ac66029, dev 1, ep 0x0, starting at offset 0x16f13f9c0
100.268804: xhci_queue_trb: CMD: Stop Ring Command: slot 1 sp 0 ep 1 flags c

Trace is a bit hard to follow as we can't distinguish between hosts.
Also seems that some events are just missing from trace.

Control transfers on ep0 fails after this.

No idea why this transfer does not complete, but I'd start by taking a better look at
the 'Context state error' responses to stop endpoint commands.

This error should mostly occur when a stop endpoint command races with an error on the endpoint,
and in these cases the endpoint state should be "error" or "halted".
In this trace endpoint state often stopped.

105.388818: xhci_queue_trb: CMD: Stop Ring Command: slot 1 sp 0 ep 1 flags c
105.389099: xhci_handle_event: EVENT: TRB 000000016f583260 status 'Context State Error' len 0 slot 1 ep 0 type 'Command Completion Event' flags e:c
105.389101: xhci_handle_command: CMD: Stop Ring Command: slot 1 sp 0 ep 1 flags c

Many other control transfer requests in this log cause a protocol stall, meaning device
doesn't support the request. xHC will halt the host side of an endpoint on both
functional and protocol stalls, and needs to be recovered with a reset endpoint command.

Maybe ASMedia host side endpoint is really halted even if it reports stopped, and needs a
reset endpoint command to recover?

Thanks
Mathias

Thanks Mathias!

I received an updated firmware image from ASMedia. It seems to improve
the 110 error problem a little bit - after the error occurs, I can
still run lsusb without it hanging. However, the streaming performance
of the camera has worsened with the new firmware; even with only one
camera connected, I get "ERROR Transfer event TRB DMA ptr not part of
current TD ep_index 8 comp_code 1" after just a few frames.

I guess the good news is these logs might be easier to analyze, since
there's only one endpoint needed to reproduce the error. I'm not sure
if this is the same behavior I was seeing before or not.

Trace is missing most events, maybe the CPU handing the xHC interrupts
is not being traced. Getting all the events would help.

> dmesg: https://bostondynamics1.box.com/s/7420hi96o5o0f8rmsc2vaafwxf8fcv9y

Combining dmesg and trace it looks like ASMedia hosts fails to create a
transfer completion event for the last normal transfer block (TRB) on the
last segment before ringbuffer wraps back and continues handling events at
the beginning of the ring.

[ 116.226252] xhci_hcd 0000:01:00.0: ERROR Transfer event TRB DMA ptr not part of current TD ep_index 8 comp_code 1
[ 116.226254] xhci_hcd 0000:01:00.0: Looking for event-dma 000000003f82b000 trb-start 000000003f82cfe0 trb-end 000000003f82cfe0 seg-start 000000003f82c000

The xHC host hardware itself probably handled the transfer as it continues
handling later TRBs on this ring (cycling back to first segment).
So driver is waiting for an event for TRB at 0x3f82cfe0, while driver is
generating events for later TRBs at:
000000003f82b000
000000003f82b010
000000003f82b020
000000003f82b030

bulk endpoint 4 i(n) has a ring buffer with two segments. each fits 256TRBs
0x000000003f82b000
0x000000003f82c000

last TRB of each segment contains a link TRB (at 0x..bff0 and 0x..cff0) that
points to next segment, link TRB on last segment has a cycle bit set.

Looks like TRBs are queued normally,
queue TRB @ 0x3f82cfe0
116.257625: xhci_queue_trb: BULK: Buffer 000000003f930000 length 32768 TD size 0 intr 0 type 'Normal' flags b:i:I:c:s:I:e:c
116.257625: xhci_inc_enq: BULK 0000000084ebe58d: enq 0x000000003f82cff0(0x000000003f82c000) deq 0x000000003f82cfa0(0x000000
...
queue TRB @ 0x3f82b000
116.259186: xhci_queue_trb: BULK: Buffer 000000003f910000 length 32768 TD size 0 intr 0 type 'Normal' flags b:i:I:c:s:I:e:c
116.259186: xhci_inc_enq: BULK 0000000084ebe58d: enq 0x000000003f82b010(0x000000003f82b000) deq 0x000000003f82cfb0

A wild guess would be that this is somehow related to the cycle bit in of the
Link TRB. Maybe ASMedia HW processes the link TRB before creating the event for
the last normal TRB, and sets the cycle bit incorrectly for it?

Just recalled this same issue was seen some time ago.

https://lkml.org/lkml/2021/4/2/197
https://patchwork.kernel.org/project/linux-usb/patch/20210416093729.41865-1-ole@salscheider.org/#24159595

Also make sure the new firmware has proper PCI ID so that xhci driver runs
the correct ASMedia quirks

This one especially:
9da5a1092b13 xhci: Bad Ethernet performance plugged in ASM1042A host

Thanks
Mathias

Ah, good catch. The PCI ID did indeed change -
New firmware:
01:00.0 USB controller [0c03]: ASMedia Technology Inc. ASM2142 USB 3.1
Host Controller [1b21:2142]
Vs. with old firmware:
01:00.0 USB controller [0c03]: ASMedia Technology Inc. Device [1b21:3042]

I'll see if ASMedia can fix this for me.

Thanks,
Joe
