Hrm crud, I've fixed this, but you may have to re-run the init's. Start with
btrfs check --repair <device>
and then see if it works. If not do
btrfs rescue init-extent-tree <device>
btrfs rescue init-csum-tree <device>
btrfs check <device>
and then you should be good to go. Thanks,
