used is reported.

After remounting with enospc_debug and rerunning btrfs balance start
-dlimit=1 and there is a ton of output...

There are no snapshots. Three subvolumes. There's nothing unusual

So it's back to functional again but does seem like some kind of bug
that it had not allocated another metadata block group sooner when it
could have. Once all the space was locked up as data bg's, it was
inevitably going to get stuck like this.

That the metadata chunks don't get allocated timely in advance is sort
of known and not fixed to my knowledge. Also, it's the reason why free
space tree is not yet default.
