x86/PV: assert page state in mark_pv_pt_pages_rdonly()

About every time I look at dom0_construct_pv()'s "calculation" of
nr_pt_pages I question (myself) whether the result is precise or merely
an upper bound. I think it is meant to be precise, but I think we would
be better off having some checking in place. Hence add ASSERT()s to
      verify that
      - all pages have a valid L1...Ln (currently L4) page table type and
      - no other bits are set, in particular the type refcount is still zero.

  commit 9186e96b199e4f7e52e033b238f9fe869afb69c7
  Author: Andrew Cooper <andrew.cooper3@citrix.com>
  Date: Thu Jun 9 14:20:36 2022 +0200

      x86/pv: Clean up _get_page_type()

Various fixes for clarity, ahead of making complicated changes.

       * Split the overflow check out of the if/else chain for type handling, as
         it's somewhat unrelated.
       * Comment the main if/else chain to explain what is going on. Adjust one
ASSERT() and state the bit layout for validate-locked and partial states.
       * Correct the comment about TLB flushing, as it's backwards. The problem
case is when writeable mappings are retained to a page becoming read-only,
as it allows the guest to bypass Xen's safety checks for updates.
       * Reduce the scope of 'y'. It is an artefact of the cmpxchg loop and not
valid for use by subsequent logic. Switch to using ACCESS_ONCE() to treat
all reads as explicitly volatile. The only thing preventing the validated
wait-loop being infinite is the compiler barrier hidden in cpu_relax().
       * Replace one page_get_owner(page) with the already-calculated 'd' already in
         scope.

      No functional change.

      This is part of XSA-401 / CVE-2022-26362.

  commit 9186e96b199e4f7e52e033b238f9fe869afb69c7
  Author: Andrew Cooper <andrew.cooper3@citrix.com>
  Date: Thu Jun 9 14:20:36 2022 +0200

      x86/pv: Clean up _get_page_type()

      This is part of XSA-401 / CVE-2022-26362.

  commit 4dc1815991420b809ce18dddfdf9c0af48944204
  Author: Jan Beulich <jbeulich@suse.com>
  Date: Fri Feb 19 17:19:56 2021 +0100

x86/PV: harden guest memory accesses against speculative abuse

      Inspired by
      https://lore.kernel.org/lkml/f12e7d3cecf41b2c29734ea45a393be21d4a8058.1597848273.git.jpoimboe@redhat.com/
and prior work in that area of x86 Linux, suppress speculation with
guest specified pointer values by suitably masking the addresses to
non-canonical space in case they fall into Xen's virtual address range.

      Introduce a new Kconfig control.

Note that it is necessary in such code to avoid using "m" kind operands:
If we didn't, there would be no guarantee that the register passed to
guest_access_mask_ptr is also the (base) one used for the memory access.

As a minor unrelated change in get_unsafe_asm() the unnecessary "itype"
parameter gets dropped and the XOR on the fixup path gets changed to be
a 32-bit one in all cases: This way we avoid pointless REX.W or operand
size overrides, or writes to partial registers.

  commit 4dc1815991420b809ce18dddfdf9c0af48944204

I hadn't notice. Thanks for highlighting. I'm kind of surprised as XenServer testing showed no major issues with stimer and indeed it is now on by default in the latest release. I'll take a look.

  commit 26fba3c85571354ecb1a1a3dde77fb38633515d3
  Author: Paul Durrant <paul.durrant@citrix.com>
  Date: Tue Mar 19 16:29:00 2019 +0100

viridian: add implementation of synthetic timers

This patch introduces an implementation of the STIMER0-15_CONFIG/COUNT MSRs
and hence a the first SynIC message source.

The new (and documented) 'stimer' viridian enlightenment group may be
      specified to enable this feature.

While in the neighbourhood, this patch adds a missing check for an
attempt to write the time reference count MSR, which should result in an
exception (but not be reported as an unimplemented MSR).

NOTE: It is necessary for correct operation that timer expiration and
message delivery time-stamping use the same time source as the guest.
The specification is ambiguous but testing with a Windows 10 1803
guest has shown that using the partition reference counter as a
source whilst the guest is using RDTSC and the reference tsc page
does not work correctly. Therefore the time_now() function is used.
This implements the algorithm for acquiring partition reference time
that is documented in the specifiction.

  commit 26fba3c85571354ecb1a1a3dde77fb38633515d3
