[GIT PULL FOR v4.8] (v2) CEC fixes, vivid CEC enhancement

These patches fix a number of bugs in the CEC framework. These were discovered
while extending and improving the compliance tests for the CEC API.

The vivid patch adds MONITOR_ALL support.
