[PATCH v2 0/9] Broxton ddi phy refactoring

Update series with review addressed. Changes since v1:

  - Use lockdep_assert_help() in bxt_ddi_phy_init().
  - Fix mangled patch.
  - Pass enum dpio_phy instead of mmio base to macroc.
