tools/cpuid: Plumb nested_virt down into xc_cpuid_apply_policy()
Nested Virt is the final special case in legacy CPUID handling. Pass the
(poorly named) nested_hvm setting down into xc_cpuid_apply_policy() to break
the semantic dependency on HVM_PARAM_NESTEDHVM.
No functional change.

This is totally bizarre.
From

we get a bunch of:
debug : libvirt_vmessage:71 : libvirt_vmessage: context='libxl'
format='%s%s%s%s%s%s'
lines, which I suspect means that libxl has tried logging and error, and
its not been rendered correctly.
The only possible change in libxl is side effects from the extra call to
libxl_defbool_val() which asserts that the boolean isn't in its default
form.
However, by this point in booting, libxl__domain_build_info_setdefault()
should have already forced it to true or false.

x86/entry: Track the IST-ness of an entry for the exit paths
Use %r12 to hold an ist_exit boolean. This register is zero elsewhere in the
entry/exit asm, so it only needs setting in the IST path.
As this is subtle and fragile, add check_ist_exit() to be used in debugging
builds to cross-check that the ist_exit boolean matches the entry vector.
Write check_ist_exit() it in C, because it's debug only and the logic more
complicated than I care to maintain in asm.
For now, we only need to use this signal in the exit-to-Xen path, but some
exit-to-guest paths happen in IST context too. Check the correctness in all
exit paths to avoid the logic bit-rotting.
