Changelog [since v4]
 - Mukesh addressed the reviewer's concerns.
 - Took Mukesh's patches and redid the changelogs.
 - Added Ack-ed as appropiate.
 - Fixed PVHVM 32-bit bootup issues.
 - Did some various cleanups, split some patches up.

The big change is that I've made the xen_remove_from_physmap structure
be of the same size and offset on 32 and 64 bit builds. Currently PVH 
only runs with 64-bit guests, but in the future it could run with 32-bit.
This change will eliminate having to add a compat layer to deal with
32-bit hypercalls.

Besides that the patches boot on an normal hypervisor - in
dom0 (32 or 64bit), PV domU (32 or 64) and PVHVM domU (32 or 64).
I don't have the PVH hypervisor patches so I could not address the
change to xen_remove_from_physmap but I hope Mukesh can do that.

PVH allows PV linux guest to utilize hardware extended capabilities, such
as running MMU updates in a HVM container.

This patch allows it to be configured and enabled. Also, basic header file
changes to add new subcalls to physmap hypercall.

Lastly, mfn_to_local_pfn must return mfn for paging mode translate
(since we let the hypervisor - or CPU - do them for us).

With the split of the 'unsigned int space' in a 'u16 space'
and 'u16 foreign_domid' in xen_add_to_physmap the compiler
won't write the full 32-bit value in 'space'. Instead
it will write a 16-bit value - which is OK. The problem
is that we allocate the xen_add_to_physmap structure from
the stack which can (and it has) various garbage on it.

The end result is that without this patch we end up
providing this to the hypervisor:

   xatp.space = 0xc1310001;
instead of:
   xatp.space = 0x1;

Forcing the foreign_domid to 0, will over-write the garbage
that was on the stack.

The PV and PVH code CPU init code share some functionality. The
PVH code ("xen/pvh: Extend vcpu_guest_context, p2m, event, and XenBus")
sets some of these up, but not all. To make it easier to read, this
patch removes the PV specific out of the generic way.

No functional change - just code movement.

Make gdt_frames[]/gdt_ents into a union with {gdtaddr, gdtsz},
as PVH only needs to send down gdtaddr and gdtsz in the
vcpu_guest_context structure..

For interrupts, PVH uses native_irq_ops so we can skip most of the
PV ones. In the future we can support the pirq_eoi_map..
Also VCPU hotplug is currently not available for PVH.

For events (and IRQs) we follow what PVHVM does - so use callback
vector.  Lastly, for XenBus we use the same logic that is used in
the PVHVM case.

.. which are surprinsingly small compared to the amount for PV.
First the set/clear mmio pte function make a hypercall to update the
P2M in Xen with 1:1 mapping. Since PVH uses mostly native mmu ops, we
leave the generic (native_*) for the majority and just overwrite the
baremetal with the ones we need.

Two local functions are introduced to add to Xen physmap for Xen remap
interface. Xen unmap interface is introduced so that the privcmd PTe entries
can be cleared in Xen P2M table.

In the bootup code for PVH we can trap cpuid via vmexit, so don't
need to use emulated prefix call. We also check for vector callback
early on, as it is a required feature. PVH also runs at default kernel
IOPL.

In setup.c which deals with E820, in xen_add_extra_mem() we can skip
updating P2M as it's managed by Xen. PVH maps the entire IO space,
but only RAM pages need to be repopulated.

Finally, pure PV settings are moved to a separate function that are
only called for pure PV, ie, pv with pvmmu.

For balloon changes we skip setting of local P2M as it's updated
in Xen. For grant, the shared grant frame is the PFN and not MFN,
hence its mapped via the same code path as HVM.

PVH only supports the batch interface. To map a foreign page to a process,
the PFN must be allocated and PVH path uses ballooning for that purpose.

The returned PFN is then mapped to the foreign page.
xen_unmap_domain_mfn_range() is introduced to unmap these pages via the
privcmd close call.

I'd also like to see at least the interface definitions in the h/v tree
if not the implementation right away.

The flip side is that we have agreed that the interfaces are not
considered set in stone / stable until we've had a chance to review the
implementation, so perhaps it is better not to commit them to
xen-unstable.hg right away.

I don't know what the right answer is. Perhaps we should at a minimum
reserve the subop numbers even if we don't yet define what they mean in
the Xen tree.

But even then - what use is it to have 3.8 possibly only work on
some intermediate (perhaps even just privately built) hypervisors?

I guess I've been told and forgotten but why does this need to differ
between PV and PVH? Can the hypervisor not take the gfn in gdt_frames[0]
= gdt_mfn and figure out the gdtaddr from it? Is this because n the PV
case the H/V loads the GDT with the address of its own mapping of the
gdt_frames but in the PVH case we have no such mapping because the
pagetables don't have a Xen region in them?

Mukesh was having trouble keeping up with rebasing both the Linux and
Xen sides simultaneously to keep up with development, so he decided to
concentrate on getting the Linux side in first.

Or an #define as a flag? Say:

#define PHYSDEV_MAP_IOMEM_ADD		(1<<1)
#define PHYSDEV_MAP_IOMEM_REMOVE	(1<<0)

which means that '0' is of course an invalid value and
alter the 'add_mapping' to a 'flag' value, as so:
