* USB 2.0 ehci failure with large amount of RAM (4GB) on x86_64

Hi,

I recently added two more memory modules to my Gigabyte K8NXP-SLI
motherboard, bringing the total up to 4GB.  I had 2GB previously and
things were running well with kernel 2.6.16.9 x86_64. The CPU is an
AMD 4800+ X2.

After the upgrade, USB 2.0 stopped working. After connecting a pen
drive the following dmesg comes up

ehci_hcd 0000:00:02.1: GetStatus port 3 status 001803 POWER sig=j CSC CONNECT
hub 1-0:1.0: port 3, status 0501, change 0001, 480 Mb/s
hub 1-0:1.0: debounce: port 3: total 100ms stable 100ms status 0x501

and then it repeats attempting different addresses.  If I rmmod
ehci_hcd, then I can get the device to work, of course it's just
running at USB  1.1.

There appear to be varying reports of this problem in bugzilla and on
message boards:

bugzilla #6201

 I am not sure which of these, if any, are the same as the problem I'm
having.  Any advice on where to go with this problem would be much
appreciated.  I suspect that a significant clue to this is that it
only occurs with large amounts of RAM. It also only seems to occur in
64-bit mode.  I booted a 32-bit kernel from the Slax live CD  5.1.0
and USB 2.0 worked fine with all my RAM intact (well only 3.4 GB, but
that's a different story). I'm happy to post more details and or try
patches out.  I just retested with kernel  2.6.16.13 and the problem
persists.

I've already tried various combinations of  settings in the BIOS, but
they have no effect.  There is an option to remap for 4GB.  If that is
turned off then the kernel only sees  3.4 GB of RAM instead of 4GB,
but it seems to have no effect on this problem.  I also tried changing
the USB memory option between shadow and lowmem.  That also had no
effect on this problem.  I do use the NVIDIA binary graphics driver,
but I retested for this problem without it loaded and the problem
persists.

Can you try 2.6.17-rc3?

Also, when booting, are there any messages about EHCI BIOS handoffs?
I've seen some reports of this in the past, but it seems to be fixed
with updated bioses.

Andi, you remember what fixed this the last time?

IIRC there wasn't a fix - just a conclusion that in one case it was likely
a bad interaction with SMM code. In the other case we didn't get that far.

I tried 2.6.17-rc3 and the problem persists.  I don't see any messages

There's an erratum in NF4 parts affecting certain EHCI accesses to
memory addresses over the 2GB mark, and you might be seeing this.

Presumably you're running with the GART IOMMU?  If not, then turn
that on.  Maybe even turn on IOMMU_DEBUG.

Another experiment might be taking the PCI_VENDOR_ID_NVIDIA case
in drivers/usb/host/ehci-pci.c and adding a call to dma_set_mask()
in addition to the existing call.

And when you tell the kernel to only use 2 GB, it works again right?
Just trying to rule out hardware or hardware-init bugs.

Maybe you could change both dma masks to be 31 bits and then force the
IOMMU active, so it gives out addresses below 2GB.  You should be able
to make it so that the EHCI controller never sees addresss with the
high bit set, making your 4 GB config act like a 2 GB one (via IOMMU).

Which one hopes means "working" ... it'd be good to know a workaround!

This isn't neceessarily a variant of that known NForce silicon bug, but you
could help confirm that by changing the "3strikes" message so it prints
out urb->transfer_dma ... and maybe make qh_completions() dump the dma
addresses of the qh and qtd too (iff urb->status becomes -EPROTO).

Thanks for all suggestions.  I tried passing mem=2048m to the kernel. 
This is with 2.6.16.13.  This did fix the USB ehci problem.  Of course
the kernel only sees half of my RAM, so this is not a satisfactory
long-term workaround.

As for your other suggestions, I'm not sure how to implement those. 
I'm not a kernel developer.  If you can give me more specific
instructions or send me a patch I would be happy to try those out. 
Otherwise, if you feel that this is unrelated to the other entries in
bugzilla with similar symptoms, I would be happy to submit a bug
report.

Like I said in my previous message, I am not a kernel developer.  I am
a programmer though, so I thought I'd give your suggestions a try.

I added 1 line to drivers/usb/host/ehci-pci.c which sets the DMA mask,
and now it seems to work with ehci loaded and with 4 GB of RAM.
Unfortunately, I don't really understand what I did.  Perhaps you have
a better idea what this is doing and if it is correct.

In my last message I sent a patch that I thought fixed the problem.

I made a mistake in my testing.  It turns out that my patch doesn't
have an effect on this problem.  What I discovered was a workaround: 
If I rmmod ehci-hcd and then modprobe it back in, usb 2.0 it works. 
That's what I did when I tested my patched version, so I mistakenly
thought it was my patch that fixed it.

So it seems that there is something that happens when the ehci-hcd
shuts down that helps initialize it to work properly.  What could it
be?

Interesting.  My guess is that the IOMMU is helping you out, and the
issue is that the silicon erratum applies to the I/O buffers too, not
just to a subset of the schedule data structures.

Can you confirm that this patch also resolves your issue?  Reboot
from scratch a few times (power off, warm reboot, etc) to be
reasonably sure you're seeing the cases that failed before.

Modify workaround for the EHCI quirk in newer NVidia controllers by
assigning the DMA mask, not just the consistent DMA mask.  It seems
to matter, as if the silicon erratum isn't quite what was described
in the original bug workaround (or there's another erratum).

Thanks for the patch.  However, did you not get my very last message?
Checking back through my mail logs, I think it might not have been
delivered.

After claiming that the dma mask fixed the problem, I quickly
discovered that the dma was not what did it.  Thus the patch you sent
does not work.

But, here is what does: If I rmmod ehci_hcd and then modprobe
ehci_hcd, it works, and I get full USB 2.0 speed.  There must be
something happening in the clean-up code of the the ehci_hcd  that
fixes whatever conflict I'm having.  When I originally made that patch
and then reloaded the ehci_hcd module to test it, I mistakenly thought
that it was the patch that was fixing the problem.  I should have
tested more thoroughly before sending the e-mail, but I got very
excited.  Sorry about that.  After several days of using the
rmmod/modprobe workaround, I do feel confident that it is a legitimate
fix.

I noticed that you added the mask inside the case while Nathan
added it outside. So, he did it for all nVidia silicon.

I would think it may be better if he tested your patch very
exactly on top of a clean kernel (e.g. make sure that his own
patch is not in the way), and also sent us the lspci output
with the chip revision numbers.

I'm having a similar-seeming issue on my nforce3 board w/ 3GB ram. I
can use my usb2 devices normally if I boot with mem=2G but if I allow
linux to see all 3GB, plugging in a usb2 hub goes like this (full
dmesg and lspci attatched):

Hi all,
I've been hitting this bug (or a similar one) for the last few months; it also 
happens on 32 bits with 1 GB of RAM. My system is a somewhat old Athlon, 
coupled with a VIA chipset and an ALI USB 2.0 PCI card.
Whenever I plug in my creative muvo tx 1GB I get the following messages:

If I rmmod ehci_hcd then ohci_hcd kicks in and it works, albeit at 12 MB/s.
I'm using the nvidia proprietary module and ndiswrapper, but removing them and 
rebooting to an untainted kernel does not change anything.
