The patch titled
     Subject: io-mapping-indicate-mapping-failure-fix
has been removed from the -mm tree.  Its filename was
     io-mapping-indicate-mapping-failure-fix.patch

This patch was dropped because it was folded into io-mapping-indicate-mapping-failure.patch
