This patch moved the ifindex and flags fields from struct mptcp_addr_info
to struct mptcp_pm_addr_entry.

It changed the type of __mptcp_subflow_connect's parameter loc from
struct mptcp_addr_info to struct mptcp_pm_addr_entry too.
