[2.6-test] Bug in usb-storage or scsi?

Hello,
I was able to access my Music Pen mp3 player using usb-storage driver
in 2.4 without any problems. After updating to 2.6 this was not possible
anymore. It seems that usb-storage driver in 2.6 detect the device but
I was unable to access /dev/sda1. "mount -t vfat /dev/sda1 /mnt" returns
this error: "mount: /dev/sda1 is not a valid block device"

Under 2.4.21-ck3, sda1 is corectly registered.

Please see the attached files containing dmesg snippets from 2.4 and 2.6
kerneles as well as 2.6 config. If you need more information I'll be glad
to provide it.

The 2.4 kernel that I tested was 2.4.21-ck3
The 2.6 kernel that I tested was 2.6.0-test5-mm1, 2.6.0-test4 and 2.6.0-test1

More problems with that stupid MODE-SENSE cache page!  There are so many 
USB storage devices that have problems with that -- I wonder if it's worth 
the effort to try to continue supporting it?

Georgi, the problem is with your mp3 player, not usb-storage or SCSI.  
It's crashing when given a perfectly legal SCSI command.  Linux 2.4
doesn't issue the command; that's why it works okay.

If you want a temporary fix for 2.6.0, you can do this:  Edit the 
routine sd_read_cache_type in the file drivers/scsi/sd.c (near line 1100).  
Get rid of (or #ifdef out) most of the function; just leave the last few 
lines where it does:

		printk(KERN_ERR "%s: assuming drive cache: write through\n",
		       diskname);
		sdkp->WCE = 0;
		sdkp->RCD = 0;

You might want to change the KERN_ERR to KERN_NOTICE.

However, you might also want to think twice before doing this if you have 
any other SCSI disks, because making this change will affect all of them.

Well it probably is tested only with windows, so it's no suprise that
device's USB implementation is buggy.

Thanks a lot! That worked fine! Now the device is detected and working.

Ugly patch is attached for reference. I hope some workaround for this kind
of buggy devices is developed in the future. Thank again.

Is there any feeling about how to handle these ongoing problems with the 
mode-sense cache page?  There doesn't seem to be any general solution that 
can work with all USB storage devices.  Some hang when asked to read the 
entire page; some hang when asked to read just part of the page; some hang 
when asked to read just the page header.

What do the SCSI folk have to say about it?

Yes, much.

Maybe more feeling if you remind/teach me what breaks if we don't fetch
page 8 re Cache.  First I imagine automounters mount read-only disks rw,
and then the write cache gets errors while flushing.  I wonder what else
breaks.  Maybe thruput varies for devices that do or do not bother to
claim they have write cache.

Maybe more feeling if you can say how in/accurate page 8 re Cache
commonly is.  For example, I know of no clear & broadly adopted standard
that requires a device to declare write-behind or background defect map
updates as a form of write cache.

I understand you to be saying we can settle for an sd solution, we don't
mind if the sd heuristic we discover then doesn't apply to pdt x05
dvd/cd.

Those lines appear in the log below.  The ... ellipses you see were
typed by me, sorry I don't have a more complete intact log, nor have I
yet repeated this result myself.

To provoke this log overnight I ran a shell script to repeat something
like:
pldd of=/dev/sg1 if=/dev/zero bs=1m

The "usb 1-1: sg_complete, unlink --> -19" appeared when or before an
ioctl SG_IO hung.  The Debug/ Call Trace appeared in response to a
`modprobe -r sg` that hung.  I'm confident the cable was not unplugged
because rebooting the system made sg work again.  I'm confident the
drive power was not cycled because reboot provoked only an SK ASC = x 6
29 Reset unit attention, without an SK ASC = x 6 28 GoneReady unit
attention.

[usb-storage] folk tell me some linux-scsi folk may appreciate me
mentioning this.

Thanks for bringing this up.

A user process is doing IO and the user decides to kill the
process while a command is "in flight". After the user hits
Ctrl+C, sg_close() is called and the sg driver lets it go
through (i.e. doesn't hold the user process in an unkillable
wait). Since the sg driver knows a command done notification
is coming (callback to sg_cmd_done()) then it keeps all its
related instances alive (including the scsi_device instance)
and notes this situation by setting its sfp->close flag.

When the callback arrives, the respone is discarded and if
this is the last outstanding command on this device then
scsi_device_put(sdp->device) is called. scsi_device_put()
isn't designed to be called from such a context. However sg
(and all other upper level drivers I suspect) have no suitable
context to call scsi_device_put() from in this situation.

Holding the sg_close() until the response arrives is an ugly
solution. Any better suggestions?

scsi_device_put() calls down_write() so it needs to be called in process
context.  Probably the best way to do this is by calling schedule_work().
An ickier solution would be to hijack an error handling thread ;-)

The down_write was recently introduced and also hurts my scsi_device
refcounting patches badly.  I'll submit a patch make scsi_device_{get,put}
nonblocking again ASAP.

mid-September, if I boot to -test4, Ctrl+C does reliably spew dmesg if
pressed during:

-test7 with much the same .config accepts Ctrl+C without complaint.

A contributing factor seems to be usb/storage injecting an unsolicited
request sense injected as if it were an auto sense, despite bCSWStatus =
x00 Passed.

Specifically I tried the following, except where you see ^D in my
terminal log I actually pressed Ctrl+D:

The reason for the unsolicited REQUEST-SENSE is that usb-storage expects
that certain commands will never yield a short transfer, so if they do it
must mean something has gone wrong, even if Status = 0.  READ-CAPACITY is
one of those commands.  Since sgp_dd asked for a 64-byte READ-CAPACITY, it
gets what it deserves.

occur only if the layers above request such an abuse - never

devices.

linux usb-storage detects that something has gone wrong.

I should prepare and forward a patch to correct sgp_dd?

We the community now broadly accept this rule?  We say apps own the job
of keeping the bus "on the thin diagonal" i.e. asking for precisely the
expected count of data bytes?

I expect we can do better, but already I stumbled into a patch that

I don't mean to be stubborn - but I do not understand.

Are you saying we have experience of devices that work Better if we the
host assault them with unsolicited request sense?  Personally I have
precisely the opposite history of pain: I know of Scsi transports that
die if abused with an unsolicited request sense.

I know bInterfaceProtocol = x01 CB pretty much requires unsolicited
request sense.  I mean to be asking about bInterfaceProtocol = x50 BBB.

Below appears a patch to make sg3_utils sgp_dd.c as polite as sg_dd.c or
sgm_dd.c.  I know now, this patch sets Hi = Di in sgp_dd.c precisely as
sg_dd.c and sgp_dd.c do.

This fixes my actual problem, but leaves me mystified over why we
include in drivers/usb/storage/ any code path that assaults a
bInterfaceProtocol = x50 BBB device with unsolicited sense.

There was at least this thread last April:

Matthew had a simple patch so that emulated hosts did not do any (sd.c)
mode-sense cache checks, James had a filter commands patch.

There were changes since then in how the MODE SENSE is handled (sd.c
only?).

In current 2.6 code, the scsi_host emulated flag does nothing, and can
be deleted.

I don't like per-host (or transport) flags or filters that cannot be
modified. They make some sense for hosts that process the scsi commands
(like a RAID card). If you moved a device from one transport to another,
the commands sent to the device should not change: for example, you move
an iSCSI attached device onto your local system via SPI.

IMO we ought to go with two new BFLAGS, one to block the MODE SENSE page 8
(for the cache), and one to block the MODE SENSE page 0x3f (to check the
write protect). 

A per host bflags could be added, and overwritten by per device (devinfo)
settings. I'd rather we black list the broken ones but that does not
appear to be practical. usb-storage could then set the per-host bflag to
include the two new flags.

We still end up sending some non-popular SCSI commands to all SCSI
devices.

Such flags would not stop sg from sending those commands, but that is
postive as well as negative in that you can override what the kernel is
doing via user space (sort of what Pat LaVarre experienced).

Also mentioned in the above thread:

The EVPD code has been removed, and is not an issue.

There is now code to set BLIST_INQUIRY_36 (we can still potentially hang
broken devices before it is set, but you should get a warning, and then
could modify devinfo on the boot/module command line or via proc devinfo).

What do people think of just having per-device flags that the host driver
could set during the slave_configure() callback?  The point of these flags
is not to prevent bad commands from being sent to the device --
user-generated commands sent via sg should always be allowed.  Rather, the
point is to prevent sd.c from generating these commands in the first
place.  (Apparently the commands don't present a problem for sr.c.)

So for example, usb-storage could set the BFLAG to block MODE-SENSE page 8
for any disk-type device.  This isn't a perfect solution; consider an
iSCSI-attached device that is actually a usb-storage disk on some server.  
Nevertheless, this might take care of the majority of the problems we see.  
(I haven't seen any MODE-SENSE page 0x3f problems, but others have.)

I'm not in principle opposed to this.  That's essentially what Andries
did for the mode sense 6 vs 10 problem.

They can't be BLIST flags, they have to be flags in the struct
scsi_device, but they're easy to add as long as we get a definitive list
of what's necessary.

James and Matt:

Please tell us what you think of the two patches below.  The first patch 
introduces 2 bitflags: one causes sd.c to skip the mode-sense request for 
page 0x08 and the other causes it to skip the request for page(s) 0x3f.

The other patch makes usb-storage set the page-8 flag for disk-type
devices.  I didn't set the page-3f flag because I haven't noticed that
causing much problems.  If Matt wants to add it that will be easy to do;
if we end up not needing it then it will be easy to remove it from the
first patch.

I have some reason to believe Windows often passes thru something like:
-x "1A 00 08 00 18 00" -i x18
i.e. a ModeSense6 of Cacheing page x08 for the whole 4 byte header plus
one whole 8 byte block descriptor plus the whole xC byte standard page.

But I haven't studied this closely enough to know if that's precisely
correct, nor do I know when it gets translated to op x5A ModeSense10,
nor do I know when it is faked out in the host entirely, never reaching
the bus.

Getting a wrong answer isn't quite as bad as crashing the peripheral's 
firmware.  My proposed solution is meant to attack the problems of USB 
mass storage devices that crash when asked to report MODE-SENSE page 8, 
possible also when asked for page 0x3f.

I imagine page x3F is an sd issue only?

I remember Ansi says the mode sense header reports writability only for
pdt x00 hdd.

I remember pdt x05 dvd/cd devices misbehaving when their available x3f
AllPages data exceeded the xFF bytes that x1A MODE_SENSE can copy,
though x5A MODE_SENSE_10 worked fine.

And I often repeat, I remember clearly that the popular OS doesn't often
ask only for a four-byte x1A MODE_SENSE header, because I saw that
actually doing that crashes Win 2K kernels up thru SP3, if Windows has
concluded the device needs the host to translate to x5A MODE_SENSE_10.

I thought in linux we have competing cdb authors on purpose.  For
example, how sr decides if a device is writable differs from how ide-cd
decides if a device is writable.  I thought that the powers that be like
linux that way.

Having multiple cdb authors necessarily means that the cdb's passed thru
one kind of transport or another do differ any time the multiple
versions of copy-edited authoring code differ.

In particular, people designing an unusual device to work with linux
have to repeat their work: once for sr, again for ide-cd, and so on.

The mode sense 6 vs 10 that James mentioned, search the code for
use_10_for_ms, used in scsi_lib.c, set to 0 in scsi_scan.c and set to 1 by
drivers/usb/storage/scsiglue.c.

More specifically, if you are using some LLDD or transport X as a bridge
to another transport Y, we might set flags - maybe what commands we can
send - relative to transport X, when they might be inappropriate for
transport Y.

Examples:

If USB (usb-storage) bridged to a remote device that is actually connected
via SPI, today we will try and send 10 byte MODE SENSE commands to the
device.

If iSCSI bridged to a remote USB device, we will only send 6 byte MODE
SENSE commands.

There are also devices that work with FCP or SPI, but those generally
don't have borken devices like the USB storage ones.

I don't get your point - I was talking about the transport (LLDD), not the
actual scsi device or upper level drivers.

Code used for different upper level scsi devices should not be duplicated,
I don't know ide or cd enough to comment in that area, except that there
is common cd code (struct cdrom_device_ops).

sr could probably do some of the same things as sd to figure out if a
device is writable, but we are best maintaining current algorithms (don't
have sr send new mode sense commands) to avoid borken devices.

Re whether a pdt x05 dvd/cd device is writable, I think I see
duplication among ide-cd, ide-scsi, and sr.  I hear ide-scsi is dying,
but that ide-cd and sr plan to continue competing.

For example:

sr.c get_capabilities decides CDC_DVD_RAM and cd->device->writeable by
fetching & parsing as raw hex the 2Ah Capabilities mode page via
scsi_lib.c scsi_mode_sense of page x2a, sending op MODE_SENSE = x1a or
MODE_SENSE_10 = x5a.

ide-cd.c ide_cdrom_probe_capabilities decides CDC_DVD_RAM (also stored
as CDROM_CONFIG_FLAGS(drive)->dvd_ram) by fetching & parsing as a
asm/byteorder.h packed struct the 2Ah Capabilities mode page via
ide_cdrom_get_capabilities via cdrom_mode_sense of page
GPMODE_CAPABILITIES_PAGE = x2a, always sending op GPCMD_MODE_SENSE_10 =
x5a.

Neither yet proceeds to try op 46h GET_CONFIGURATION to discover the
other six writable mmc profiles.

also seen the off-by-one slight overestimate and the high-byte-zeroed
dramatic underestimate.

Yes, but t10 mmc often quietly contradicts & overrides t10 spc for pdt
x05 dvd/cd and for atapi pdt x00 hdd/rmb.  For t10 newbies wanting to

The pernicious idea of zeroing the scsi revision in byte 2 of op x12
inquiry forms part of how mmc quietly redefines spc.  Misapplying the
sff precursor of the pdt x05 mmc spec to pdt x00 devices is part of how
msft whql qualified atapi pdt x00 devices for sale thru oem's in the
late 1990's.

In the sd world, usb hdd/rmb (pdt x00) may handle specifically the mode
sense of page 8 that win sends.

For usb bInterfaceSubClass = x06 TransparentScsi that cdb might be:
x 1A 00:08:00 18 00

For usb bInterfaceSubClass != x06 TransparentScsi that cdb might be:
x 5A 00 08:00:00:00 00 00:1C 00

usb dvd/cd (pdt x05) sell into the sr/ide-cd world instead, of course.

Tell me we care to hear specifically what some win sends to some of my
usb hdd/rmb, and I'll go collect a few sample usb bus traces.  I do NOT
know that the above cdb's are correct.  Those are just my guesses,
summing up little clues from years of messing around near here.  I hear
in particular that win 9x/me differs from win 2k/xp near hear.

I thought "everyone knew" sanest was to always fetch header + block
descriptors + whole pages?

Trouble is, that's a circular definition in that the host can't know
whether the device will volunteer an optional block descriptor or not. 
mmc says shall not, spc says maybe not but should.

We only have to care if we have to know how many Data bytes to expect to
copy In.

response to a pin 1 reset issued without op x35 sync cache.  I've heard

We know that isn't talk like windows.  msdn.microsoft.com tells us the
win 2k kernel crashes if a root-privileged app asks for just the 4 byte
header from devices for which Win translates to x5A MODE_SENSE_10 from
x1A MODE_SENSE  Because the win 2k kernel crashes so reliably, we can
confidently guess no working win 2k apps talk that way.

I get 0.5 KiB/block.  Did I fail to find an option or an appropriate
kernel patch, I do I have to write my own patch?  I'm guessing I'll find
2 KiB/block easy to fake, since 0.5 KiB divides evenly into 2 KiB.

Specifically, I want to see 2048 where now I see 512 in response to my
query `blockdev --getss`.  A more complete example console log follows:

I wonder if this "2 KiB/block loopback found where" thread belongs on
linux-kernel, rather than on linux-scsi, since the code involved seems
to be drivers/block/loop.c

I wish in the Subject line had mentioned so far I've tried writing GiB
only via usb where it dies, not elsewhere.  Today I returned after
leaving a drive idle for some days.  Nothing had appeared in the `dmesg`
meanwhile, but trying to talk to the drive provoked:

scsi: Device offlined - not ready after error recovery: host 0 channel 0
id 0 lun 0

Now commonly in -test6 I see such seemingly spontaneous events as:

Did I change some other significant variable?  Could be ...

I will rebuild my usb trace infrastructure and report again.

With usb2 in linux 2.6.0-test6 just now, ...

I find what works well for me is /etc/grub.conf:

title Red Hat Linux (2.6.0-test6)
        root (hd0,8)
        kernel /vmlinuz-2.6.0-test6 ro root=LABEL=/
        initrd /initrd-2.6.0-test6.img

What chokes often is:

title Red Hat Linux (2.6.0-test6)
        root (hd0,8)
        kernel /vmlinuz-2.6.0-test6 ro root=LABEL=/ hdc=ide-scsi
        initrd /initrd-2.6.0-test6.img

Having hdc=ide-scsi matter I think is a clue.

I don't yet know if this apparent correlation will hold up or not.

Yeah, ide-scsi is broken so you probably shouldn't be using it.

Yes, there is IMHO. The Onstream tape driver (osst) depends on ide-scsi
for controlling the IDE version of the drive (DI-30). 

Fixing ide-scsi is not trivial, though. I've spent some time on it
on and off and have not been all that successful. The biggest problem
I have is the error recovery -- making ide subsystem error handling
happen when the scsi error handler needs it...

Alan said (a few months back) that he was fixing ide-scsi in 2.4.x-ac
and that it should be forward-ported to 2.6.x, so that is where I
would begin...  I haven't checked it for progress yet.

2.4 and 2.6 also uses different error handling for ide-scsi, so it would
probably not have helped you a lot.

If that is reality, but not what we want to say, then we can balance the
work involved in updating ide-scsi vs. the likely pain of answering all
the later newbies who come here saying I swapped 2.6 into my Red Hat/
etc. 2.4/ 2.5 and life went downhill from there.

Myself I gathered 2.6 experience without ide-scsi only by luck.  I have
a usb drive and an atapi drive, I had to choose one or the other to
begin with, I happened to choose the usb drive, and I happened to
dislike the idea of overriding a kernel default.

I'm no longer sure I remember exactly how I fell back into trying
hdc=ide-scsi.  I know I'm contact with other newbies who take more the
attitude of learning to change as little as possible in how Red Hat
boots, rather than the attitude of learning how to survive with as
little from Red Hat as practical.
