More complete version of intel_dbuf_state. We finally get rid of
distrust_bios_wm and all the uglyness surrounding it. And we no
longer have to know ahead of time whether the duf allocation
might change or not, and thus don't need to pull in all crtcs
into the state up front. Now we just compute the new dbuf
state, and if it changes the affected crtcs get added to the
state naturally.

+ a bunch of cleanups.
