The dev_pagemap is a growing too many callbacks.  Move them into a
separate ops structure so that they are not duplicated for multiple
instances, and an attacker can't easily overwrite them.

The dev_pagemap is a growing too many callbacks.  Move them into a
separate ops structure so that they are not duplicated for multiple
instances, and an attacker can't easily overwrite them.
